Beneficiary Designation for Retirement Plans or Life Insurance Policies

Contact the manager of your IRA, 401(k), other retirement plan, or life insurance policy and ask them to name First Book as a full or partial beneficiary of your plan or policy.

Wills or Living Trusts

Share this sample language with your attorney or estate planner:

General Bequest (a specific dollar amount)

“I give to First Book, a nonprofit organization located at 1319 F Street NW, Suite 1000, Washington, DC 20004 (Tax ID: 52-1779606), the sum of $___________ to be used for its general purposes.”

Percentage Bequest (a percentage of your estate)

“I give to First Book, a nonprofit organization located at 1319 F Street NW, Suite 1000, Washington, DC 20004 (Tax ID: 52-1779606), ________% of my estate to be used for its general purposes.”

Residuary Bequest (what remains after other gifts)

“I give all [or a percentage] of the rest, residue, and remainder of my estate to First Book, a nonprofit organization located at 1319 F Street NW, Suite 1000, Washington, DC 20004 (Tax ID: 52-1779606), to be used for its general purposes.”

Contingent Bequest (if your primary beneficiary cannot accept the gift)

“If [name of primary beneficiary] does not survive me, I give to First Book, a nonprofit organization located at 1319 F Street NW, Suite 1000, Washington, DC 20004 (Tax ID: 52-1779606), the sum of $___________ [or other property] to be used for its general purposes.”
